The Role:
We are seeking an experienced and motivated Primary Health Network Team Lead to provide operational and clinical leadership to a multidisciplinary team delivering care for people with long-term conditions.
In this pivotal role, you will oversee day-to-day team operations, ensuring care is delivered efficiently, safely, and in line with best practice. You will play a key role in strengthening relationships with general practices, acting as a trusted advisor and connector to ensure seamless, coordinated care across services.
You will also support general practices with clinical guidance, funding and claiming processes, and contractual requirements, while linking them into THINK Hauora’s wider support systems.
You will be responsible for:
- Lead and manage a multidisciplinary team to deliver high-quality, evidence-based care for patients with long-term conditions
- Oversee daily operations, including coordination of workflows, monitoring performance, and addressing operational challenges
- Build and maintain strong relationships with general practices and healthcare providers to ensure integrated, patient-centred care
- Support and guide general practices with clinical advice, funding opportunities, and contractual obligations
- Facilitate communication, referrals, and warm handovers to ensure continuity of care
- Monitor clinical quality through care plan reviews, audits, and continuous improvement initiatives
- Contribute to financial and contract performance, including supporting reporting and resource optimisation
About You
You are a confident and collaborative clinical leader who thrives in a fast-paced, relationship-driven environment. You bring a strong understanding of primary health care and are passionate about improving outcomes for communities.
You will have:
- Bachelor’s degree in Nursing with a current practising certificate (prescribing qualification preferred)
- Minimum 10 years’ clinical nursing experience in primary health, including practice management experience
- Proven experience leading and managing multidisciplinary teams
- Strong clinical leadership skills and the ability to support and develop others
- Excellent communication and relationship-building skills
- Sound understanding of funding models, claiming processes, and contractual requirements
- A commitment to equity and improving health outcomes for Māori and other priority populations
