THINK Hauora delivers a range of services and programmes to people living within our rohe.

Many of them are aimed at helping people manage long-term conditions, and to make healthy, positive impacts on their life.


An ongoing, long-term, or recurring condition can have a significant impact on a person's life. Our services can provide people with the knowledge, skills and support they need to confidently managed their health on a day to day basis.

Some of the other services delivered via THINK Hauora include:


  • Community Cardiac Rehabilitation Programme – A group-based service to support people to get back to living a full and active life after a heart event or diagnosis. Information sessions are provided by specialists such as clinical dietitians, registered nurses and mental health clinicians who specialise in supporting people with long terms conditions and clinical exercise physiologists
  • Respiratory Physiotherapist Service – A personalised programme for managing symptoms, improving exercise levels and ensuring you can ‘sneeze with ease and cough with confidence. 
  • Retinal Screening Service – People with diabetes are at risk of developing eye damage called diabetic retinopathy. With early diagnosis and treatment, damage can be reduced or avoided.  When a person has been diagnosed with Type 2 diabetes, the General Practice Team will refer them to an Optometrist to monitor any changes. This examination should be completed every 2 to 3 years.
  • Smoking Cessation - Te Ohu Auahi Mutunga (TOAM) Stop Smoking Service is engaged to reduce smoking rates in our region to five percent by 2025. TOAM works with THINK Hauora to run Stop Smoking Clinics in General Practices. Matanga (coaches) provide a culturally responsive service to Maori, Pasifika and pregnant women. These people are supported through a four-week programme to quit smoking which is a combination of Nicotine Replacement Therapy, personal coaching and 'Vape to quit' for those using e-cigarettes.  Ask your General Practice team for a referral or call TOAM direct on 0800 40 50 11.
