Enrolling with a General Practice team

New to the district? Not seen a healthcare provider in years?  Enrol with a GP team

Are you looking to enrol with a General Practice team? Check out our open General Practice teams , email us at register@thinkhauora.nz   or talk to us on 0800 766 255 Option 1 for advice and support.

Enrolling helps you build a relationship with a General Practice team who can become your key contact point for your primary health needs and can connect you with other health services across the district. Enrolling is free, can save you money and help keep you and your whānau stay well.

Enrolment at a General Practice can provide you and your whānau with significant benefits, including:

Continuity of Care

Continuity of care means you can visit the same General Practice team over time who will get to know you and your whānau.

Access to your Health Information

Your General Practice team will hold your health record at the practice and will have your health information on hand every time you visit. This helps your team make fast, informed decisions about your healthcare when you need it. They may also share information such as medication, allergy and lab tests information with you via a secure online patient portal.

What is a Patient Portal?

Patient Portals are like personal online banking for your health and can you access to your healthcare team and your personal health information.

The patient portal can help you and your whānau can become more involved in decisions about your care. These are an easy and safe way to communicate with your primary health care team without visiting or phoning the clinic. You will still have a valuable face-to-face relationship with your primary care team.

Most general practices in our district offer portals known as ‘myIndici’ and ‘Manage My Health’. Each General Practice might run their portal slightly differently. It is likely you can:
  • view your doctor’s or nurse’s availability and book an appointment online.
  • send messages
  • receive information on repeat prescriptions
  • receive information immunisations and vaccinations
  • and/or check recent lab test results.
The ‘myIndici’ patient portal also has an exciting feature called the ‘Shared Care Plan’. With the ‘Shared Care Plan’ you and your doctor/ nurse practitioner can invite other members of your health care team, such as a district nurse, physiotherapist, or regular carer, to view and contribute to your personalised health care plan. People living with long term conditions and/or those who see many different health professionals will really experience benefits from the indici Share Cared Plan as it reduces the need to repeat the same health information at every visit!

Regular Check-ups

When your health team know you and have access to your health information they can work with you to keep you up-to-date with any immunisations, screening programmes or health checks to help you and your whānau stay well.

Lower Cost Visits

General practices normally charge a higher fee, often called a casual rate, for people who are not enrolled at the practice. If you enrol with a general practice, your care will be subsidised at that practice and you will have lower cost for clinic visits. You may also entitled to a Community Services Card to help you and your family with the costs of health care through the Community Service Card (CSC) Scheme.
