How We Work

We deliver our services to the community in two ways to help our community achieve wellbeing. 


THINK Hauora serves the primary health care needs of the MidCentral districts of Manawatu, Otaki, Tararua and Horowhenua by supporting General Practice teams. 


Primary health care covers diagnosis and treatment, health education, counselling, disease prevention and screening through General Practice teams, Iwi and Māori providers and THINK Hauora clinics to ensure our whānau stay well.   Our services include Hauora Māori, Pasifika health services, immunisations, cervical screening, sexual and reproductive health, services for older people, translation services and mental health and wellness services. 

Language Assistance

Translation Services

If you need, or someone you know needs,  language translation support, please speak with your General Practice about how they can help you.  THINK Hauora supports General Practices, optometrists and counselling consultations to offer translation services to patients who need it.  Translation services are available between 9am-5pm on weekdays. Some after hours service may be available. 

Hauora Māori 

THINK Hauora supports services that offer culturally-appropriate care, described below.  The key principles of these services include: 

 

  • Acknowledging Te Whare Tapa Whā model 
  • Clients are welcome to have whānau involved in assessment, treatment and decision-making
  • Cultural impact on assessment and treatments are considered for any health issue 
  • Engaging Whānau Ora principles, Te Ara Whānau Ora and Navigators where appropriate 

 

Te Ohu Whakaaraara

Te Ohu Whakaaraara service is available for everyone. This service uses Whānau Ora Navigators/ Kaiwhakaaraara who work alongside whānau to:

 

  • identify their aspirations, strengths and chosen pathway
  • offer coaching, advocacy and brokerage services. 

 

Whānau Ora Navigators use a Te Ara Whānau Ora strength-based process to identify whānau goals and aspirations. Whānau are supported to look at areas of concern in their lives by tapping into and reinforcing their own individual and collective knowledge and strengths and a plan is created to support whānau to excel.

Iwi and Māori Providers

Iwi and Māori providers work across the health, social and community space on various initiatives to improve the health of our whānau Māori. For more information on Iwi and Māori provides across the MidCentral district please click here

Pasifika Health 

Talofa lava, Ni sa Bula Vinaka, Malo e Lelei, Kia Orana, Taloha Ni, Fakalofa Lahi atu, Noa’ia ‘e mạuri, Tena Koe and Warm Pacific greetings. 


Our Pasifika nurses and community support workers help reduce barriers and increase access to health and other services and support Pasifika people and families to make good decisions about their health and wellbeing. Our services include: 

 

  • health coaching (exercise, diet and nutrition)
  • assistance with clinical appointments
  • eczema and asthma
  • pregnancy, breastfeeding and women’s health
  • cervical screening 
  • long term conditions support (Diabetes, high blood pressure, cardiac disease)

Pasifika Community Initiatives

PolyPaddles is a six-week water survival swimming programme run by Niuvaka Trust for children aged 6−10 years running at CLM Lido (Palmerston North) and Levin Aquatic Centre (Horowhenua).
