Health Care Home achievements for Kauri HealthCare and Tararua Health Group

Kauri HealthCare and Tararua Health Group have achieved important milestones as part of the Health Care Home Model of Care. 

Congratulations to Kauri HealthCare for achieving Health Care Home Recertification at a national level. The key to their success was enabling open notes for their patients.


Congratulations also go to Barraud Street Health Centre (Tararua Health Group) for achieving Health Care Home Certification.


The Health Care Home Model of Care incorporates whakawhanaungatanga (creating a relational model) in the delivery of care. Relationship centred care creates better health outcomes for whānau which can be as simple as ensuring that practice information resonates with people in terms of language and visual presentation or enhancing the cultural skills and competencies of staff, including understanding the unconscious bias inherent in many public services.


Our Health Care Home team work with a variety of practices across our rohe on continuous quality improvement. This model of care aims to make improvements within four key areas of general practice:


  1. Urgent/unplanned care (triage process, same day access, wait times)
  2. Proactive care (risk stratification, care planning)
  3. Routine and preventative care (telehealth, patient/whanau engagement, patient portal)
  4. Business sustainability (workforce development, staff training, lean, business processes)


There are three milestones of achievement for Health Care Home – credentialing (local), certification (national) and recertification (national).
