We’re looking for a Pulmonary Rehabilitation & Diabetes Management Coordinator to join our team at THINK Hauora. You’ll coordinate and deliver engaging, patient-centred programmes that support people living with respiratory conditions, diabetes and other long-term health conditions to better understand, manage and take control of their health.
From facilitating group sessions and one-to-one education to working alongside our multidisciplinary team and connecting people with other services, you’ll have plenty of opportunities to make a meaningful difference.

Whats the mahi?
This is a role with plenty of variety and plenty of people connection. You’ll coordinate and deliver pulmonary rehabilitation and diabetes management programmes while working alongside patients, whānau, general practices and our wider multidisciplinary team.
You’ll help people build confidence in managing their health and turn health goals into practical, achievable steps. You’ll also have the opportunity to take your mahi beyond the clinic - delivering education in community settings, gyms, homes and virtually.
You’ll get to:
- Support patients and whānau to build confidence, develop self-management skills and make positive changes to their health and wellbeing.
- Facilitate individual and group education for people living with respiratory conditions, diabetes and other long-term health conditions.
- Use health coaching and motivational interviewing to support sustainable lifestyle and behaviour changes.
- Connect people with the right support, including other health professionals, services and self-management programmes.
- Work collaboratively across the health system to support coordinated, patient-centred care and seamless referral pathways.
- Contribute to improving our programmes by using patient feedback, data and evidence to shape how we deliver our services.
About you:
We’re looking for someone who is passionate about supporting people to live well with long-term health conditions and who can bring both clinical expertise and a great people-focused approach.
You’ll ideally have:
- Registration as a health professional with a current Annual Practising Certificate (APC)
- A relevant university degree and experience and/or a tertiary qualification in Sports Science
- Experience working with people living with long-term health conditions
- Strong communication and relationship-building skills
- Confidence facilitating individual and group education
- A current, clean full New Zealand Driver Licence
- Current CPR and First Aid certification
