The Role

Are you passionate about making a real difference in people's lives? We're looking for an organised, proactive and relationship-focused Programme Co-ordinator to join our Mental Health and Wellbeing team.

This is a varied and rewarding role where you will coordinate key aspects of the Te Ara Rau Access & Choice Programme, working closely with Specialist Providers of Brief Therapy (SPBT), primary care providers, internal teams and external stakeholders. You'll play an integral role in supporting providers, coordinating referrals, delivering workforce development initiatives, meeting reporting requirements and continuously improving systems that help deliver high-quality, equitable mental health services.

You'll enjoy:

• Supporting people and whānau living with diabetes through specialist assessment, care planning, education and self-management support.
• Working alongside General Practice teams, iwi/Māori providers, community services and secondary care to deliver coordinated, person-centred care.
• Sharing your knowledge through clinical leadership, mentoring and education to strengthen diabetes care across primary care.
• Helping shape and improve diabetes services through quality improvement initiatives, collaborative clinical pathways and evidence-based practice.
• Building strong relationships that improve access to care and health outcomes across our communities.

What you'll do:

• Build strong relationships with SPBT, supporting their onboarding, education and ongoing success.
• Coordinate referrals, reporting, claims and programme administration to keep services running smoothly and efficiently.
• Lead and coordinate workforce development initiatives, including education and training with providers and partner organisations.
• Identify opportunities to improve systems and processes, helping us continuously enhance the services we provide to our communities.
