The Role

If you're passionate about diabetes care and enjoy working alongside people, whānau and healthcare providers to achieve better health outcomes, we'd love to hear from you.

As our Clinical Nurse Specialist – Diabetes, you'll provide advanced diabetes nursing expertise across primary and community care, supporting people and whānau with complex diabetes-related needs through specialist assessment, care planning, self-management support and clinical coordination. This is a permanent, part-time position of twenty-four (24) hours per week (0.6 FTE).

No two days are the same. You'll work alongside General Practice teams, iwi/Māori providers, community services and secondary care, sharing your expertise, supporting clinical decision-making, and helping strengthen diabetes care across the region. You'll also provide clinical leadership, mentoring and education that builds capability across the wider health workforce and contributes to equitable, evidence-based care.

Based in our Palmerston North office, this role includes travel across the Horowhenua, Manawatū and Tararua districts.

You'll enjoy:

• Supporting people and whānau living with diabetes through specialist assessment, care planning, education and self-management support.
• Working alongside General Practice teams, iwi/Māori providers, community services and secondary care to deliver coordinated, person-centred care.
• Sharing your knowledge through clinical leadership, mentoring and education to strengthen diabetes care across primary care.
• Helping shape and improve diabetes services through quality improvement initiatives, collaborative clinical pathways and evidence-based practice.
• Building strong relationships that improve access to care and health outcomes across our communities.

You will have:

• Registered Nurse with a current Annual Practising Certificate and a minimum of five years' nursing experience in Primary Health and/or diabetes specialty.
• Demonstrated experience providing specialist diabetes care and education, aligned with the National Diabetes Nursing Knowledge and Skills Framework (NDNKSF).
• Strong clinical leadership, communication and relationship-building skills, with the ability to work collaboratively across General Practice teams and other healthcare providers.
• Senior/Expert level Professional Development and Recognition Programme (PDRP) Portfolio (Clinical Practice), or be working towards this within an agreed timeframe. Designated Prescriber status (or working towards authorisation) is desirable.
