Whakapai Hauora is seeking a compassionate and experienced Whānau Ora Long Term Conditions Nurse to join our team. This role is based in our in General Practice and includes outreach community clinics at our offices in Foxton (Heartlands).

Key Responsibilities Include:

• Deliver Whānau Ora Long Term Conditions care.
• Conduct health assessments and develop care plans.
• Provide health promotion and disease prevention advice to patients and whanau.
• Support patients with chronic conditions, diabetes, asthma, and hypertension.
• Work collaboratively with our wider health team.
• Maintain accurate patient records in line with professional and legal standards.

Requirements

• Registered Nurse with a current Practicing Certificate.
• A willingness to work with Long Term Conditions patients.
• Strong communication and interpersonal skills.
• Ability to work independently and within a multidisciplinary team.
• Driver’s license.
