Located in the Palmerston North site at our THINK Hauora office, we have an exciting opportunity for a Community Clinical Nurse - Long Term Conditions to focus on supporting the Midcentral region Pasifika population.
As this role will focus on Pasifika, it will leverage your excellent understanding of primary (or secondary) health care and population health particularly aligned to the Pasifika population. You will have a commitment to providing high-quality health services that meet the needs of the Pasifika community. Ideally, you will be fluent in at least one Pasifika language and have an exceptional understanding of how a Pasifika culturally appropriate service influences health care provision.
This role will be varied and there is an expectation of working outside of normal hours including weekend work when requested. It will see you assessing, supporting, and managing LTCs to individuals and their whanau throughout the community. This is achieved through home visits and on-site clinics to increase accessibility to our health care services. You will work in collaboration with general practice teams (GPTs), specialist services at Midcentral Health, Pasifika trusts and organisations, and other healthcare providers in the community.
You will have the opportunity to utilise your cardiac, respiratory and diabetes experience and assist the Pasifika team in delivering/facilitating health promotion and educational programmes. You will also have the opportunity to work closely with other THINK Hauora clinical teams as well as with the specialist nurses within the organisation who will provide nursing support to enable you to expand your long-term condition knowledge.
