The Role:

We are seeking a motivated and experienced RN to join our team as a Primary Secondary Care Interface Co-ordinator. In this important position, you will play a vital role in streamlining the pathway between primary and secondary care, acting as a clinical interface and care navigator.

 

This is a pivotal role designed to bridge service gaps, enhance communication across healthcare settings, and ensure vulnerable and high-needs populations receive timely, coordinated, and culturally responsive care. You will work closely with general practices, hospital teams, and community providers to ensure patients (particularly those with long-term conditions, vulnerable populations and unenrolled individuals) are supported throughout their care journey.

 

You will be responsible for:

  • Enhancing integration between primary and secondary health services
  • Identifying and supporting patients at risk of falling through the gaps post ED presentation or hospital discharge
  • Facilitating seamless transitions of care from hospital to the community
  • Supporting enrolment of unenrolled individuals within the THINK Hauora network
  • Providing clinical case management and coordinate appropriate referrals
  • Delivering high-quality nursing care in collaboration with multidisciplinary teams.
  • Providing culturally tailored, equitable care for Māori, Pacific and priority populations

 

What we are looking for:

  • Current Annual Practising Certificate as an RN
  • Proven experience in primary care, secondary care, or community nursing
  • Strong communication and coordination skills
  • A patient-centered approach with a focus on health equity
  • Knowledge of local health systems, referral pathways, and community support services
