Improving Our System


Primary health care covers a broad range of health services, including diagnosis and treatment, health education, counselling, disease prevention and screening. A strong primary health care system is central to improving the health of all New Zealanders and reducing health inequalities between different groups. 

THINK Hauora provides primary health services both directly, and through our contracted providers (such as general practices). These services are designed to improve the health of the PHO enrolled population as well as ensuring peoples’ health is restored after illness.

THINK Hauora and general practices work together to continuously improve our primary health system for people in the MidCentral district. Some of the strategic focus areas that direct this work are;

•    Improving access and equity

A fundamental aspect to Improving Our System is tackling Health Inequities. These are health differences in a population that can be traced to unequal economic and social conditions that are systemic and avoidable; thus, being inherently unjust and unfair.

We know Māori and Pasifika communities across the region have significantly higher health and social needs. An example of how we tackle health inequities in the Manawatu is through our regions Māori Health Strategic Framework – Ka Ao Ka Awatea and the Pasifika Health Service Action and Investment Plan;
•    Supporting clinical innovation (Co-designing integrated care for priority populations) 

We are committed to a system that appropriately meets the needs of each individual and whanau through planned proactive care for long term conditions and disabilities; timely and accessible acute care when you need it and comprehensive preventative care to promote health and wellbeing (see Health Care Home tab). 

•    Supporting technical innovation (Activating smart systems) 

The MidCentral region, including THINK Hauora and general practices, is currently undergoing an exciting period of digital transformation. For health care users this means that your health care records will be available to more members of your health care team. Everyday activities such as referrals between health care services and the scheduling of appointments will use digital technologies to ensure faster processing and co-ordination between departments.
